After scheduling your appointment, new patients will receive an email link to your patient portal. The link will expire in 3 days. Please check your email for this link to your patient portal to update your records prior to coming to your appointment. If your email link has expired, please call the office to request a new link to be emailed to you.

Established patients can login to your portal at any time to update your information. Please do so prior to your appointment.

The patient portal update is the most efficient way to update your information. If you are unable to login to your portal, you can download and print a copy of the registration forms and fill them out prior to your appointment. Doing so prior to your visit will help speed up the new patient registration process. We must first know which doctor or office you will be visiting in order to provide the correct forms.

If you download forms, please be sure to bring them with you to your first visit. You will also need to bring any diagnostic films or other test results that may have been ordered or performed by another provider.

It is highly recommended and much more efficient to use the patient portal to update your information prior to your appointment.

How do I request my medical records?

Once you enter your email, you’ll receive an email with HealthMark to log in (no username or password required!). Click on the “submit request” button and follow the prompts from there. You’ll receive an email as soon as your records are available for download.

Do I have to remember another username and password?

Nope! HealthMark’s Request Manager uses email verification and secure links to get you your records quickly, efficiently and securely – and without yet another username and password to remember!

How long does it take to process requests?

Most record requests are processed within 8 business hours. So, one or two business days after you submit your request, your records will be delivered electronically – right to your inbox.
